Appointment should be made at least one week in advance. For urgent visit please make sure to call early in the morning.
Patients are required to notify the office staff 24 hours prior to their scheduled appointment for cancellation. Three ‘no shows’ may lead to termination from our practice, as we believe that numerous ‘no shows’ reduce the quality of our patient care.
Patients may choose a Resident Physician or one will be assigned to them. The turnaround of the residents is yearly. Each resident is here for a total of three years.
New patient will be asked to complete a patient information form. Every patient will be asked to complete a patient self-assessment form. Therefore we advise patients to come 30 minutes earlier than their scheduled appointment.
