We accept all insurance.
It is the patient’s responsibility to provide the correct insurance information. Please have your current insurance cards with you at each visit. All co-pays and deductibles will be collected at the time of patient’s visit.
If the patient does not have insurance, a payment plan will be implemented. Please ask our staff about the Patient Assistance Program available through the United Hospital Center.
